Workers Compensation

 

If you incur a work injury, you must report your work injury to your immediate supervisor AND Danning Chen at 215-750-2800, Ext. 4016 on the day that the injury occurred.  If the work injury occurred after normal business hours, you must report your work injury the next business day.

 

You must treat with a panel provider for 90 days from the first date your first treatment or your medical expenses may not be covered.

 

You must contact Danning Chen at 215-750-2800, Ext. 4016 to obtain and complete your Workers' Compensation paperwork in order for your claim to be filed.

 

You must notify Danning Chen in the Business Department AND your Claims Representative at Excalibur Insurance Management LLC immediately when you receive a return to work date.
